Referrals

Health professionals, care coordinators, case managers and approved service providers may use this form to refer a client to Vital Being.

Referrals may be submitted for lymphoedema, lipoedema, chronic oedema, persistent swelling, compression support, oncology-informed care, remedial massage or therapeutic massage.

Please include the client’s contact details, relevant health information, reason for referral and any funding or approval requirements.

Submitting a referral does not confirm an appointment. Urgent or emergency concerns should be directed to the client’s GP, treating medical team or emergency services.
